There is no single EKG certification, and the three that matter test very different things. The CCI Certified Rhythm Analysis Technician is 76% rhythm interpretation and is the monitor tech credential. The CCI Certified Cardiographic Technician is 62% interpretation and is aimed at people running ECGs, Holters and stress tests. The NHA Certified EKG Technician is only 24% interpretation, with 44 of its 100 scored items on acquisition and 32 on safety and compliance, and it is the entry-level credential attached to most training programs. Both CCI exams cost $175. If you sit in front of telemetry screens for a living, the CRAT is the one that matches your job.
The reason this question is confusing is that employers do not ask it clearly. A job posting will say "EKG certification required" or "monitor tech, certification preferred" and leave you to work out which of three credentials from two organizations they mean. Meanwhile every study guide on the internet is a pile of rhythm strips, which quietly implies all three exams are rhythm exams. One of them barely is.

Those interpretation percentages come straight from the published blueprints, and they are the most useful numbers in this article. On the CRAT, analyzing abnormal rhythms is 60% of the score by itself and analyzing normal rhythms adds 16%. On the CCT, rhythm analysis is 40% and analyzing the 12-lead ECG is another 22%. On the NHA CET, all of EKG analysis and interpretation is 24 items out of 100.
Which one should a monitor tech get?
The CRAT, in almost every case. It is the only one of the three built around the work a telemetry or monitor tech actually does, which is watching rhythms and escalating the ones that matter. The blueprint reflects that with almost no dilution: beyond the three quarters devoted to rhythm analysis, the remaining quarter splits across initiating monitoring services at 9%, administering cardiac tests at 9% and processing findings at 6%.
That concentration cuts both ways. A narrow exam is easier to study for and harder to bluff. You cannot pick up 20 points on the CRAT by knowing HIPAA cold, the way you partly can on the CET. Either you can look at a strip and name what it is under time pressure, or you cannot.
The one situation where a monitor tech should take something else first is when an employer or a training program has already specified the CET, which happens often because NHA has deep relationships with vocational programs. Take the exam your paycheck depends on. Just do not assume it prepared you for the CRAT later, because it prepared you for roughly a third of it.
Which one should an ECG or stress test tech get?
The CCT. It is explicitly described as being for professionals working in ECG, Holter monitoring and stress testing, and its blueprint spreads across the whole workflow rather than concentrating on strips: pre-procedural activities at 10%, performing resting ECGs at 14%, analyzing 12-lead ECGs at 22%, stress tests at 10%, ambulatory monitoring at 4% and rhythm analysis at 40%.
One thing worth flagging, because it catches people who reason from their job description: ambulatory monitoring is only 4% of the CCT. Technicians who spend most of their week hooking up and scanning Holter monitors sometimes assume the exam will reward that. It will not. The exam rewards reading tracings, and it rewards it six times more heavily than the Holter work.
Do you need a training program to sit these exams?
For the CCI credentials, no, and this is the pathway most people never hear about. Beyond a high school diploma or general education diploma, CCI accepts three qualification pathways for both the CCT and the CRAT. One is being enrolled in or a graduate of a cardiovascular or allied health training program. One is having completed a graduate or undergraduate degree in a science or physical health related field. The third is current or previous employment in cardiovascular technology or in an allied health field recognized by the American Medical Association.
That employment pathway is the door. It is supported by an employment verification letter that states your period of employment, your primary duties and your clinical competence in the field. If you have been working as a monitor tech, a nursing assistant or in another allied health role and your supervisor will write that letter, you can register without ever enrolling in an EKG program. For a working tech, that turns a several-thousand-dollar certificate program into a $175 exam fee.
What the fee actually buys, and what happens if you stall
CCI charges $175 for either exam and states that every exam fee includes a $100 non-refundable filing fee covering application review. The practical version of that sentence: if you change your mind inside your first 90 day window and request a refund, you get $75 back, not $175.
The 90 days is the part that costs people money. CCI issues an Authorization to Test with a 90 day window, and you may request exactly one extension, for another 90 days. Miss both and the fee is forfeited and you start the application over. Scheduling happens at Pearson Professional Centers subject to seat availability, so leaving it to the last fortnight of a window is a genuine risk rather than a theoretical one.
After you pass, CCI credentials renew on a three year cycle with continuing education and a renewal fee listed at $165 in CCI documentation. Since the December 2016 renewal cycle CCI has used an attestation model, so you attest to completing your continuing education rather than uploading documentation with every renewal, while remaining subject to audit.
What the exams look like, and one format detail that matters
Both CCI exams are 130 questions in two hours, with 110 scored and 20 unscored pretest items mixed in that you cannot identify. That is about 55 seconds per item, which is comfortable for rhythm recognition and tight if you are deriving answers rather than knowing them.
The format detail worth preparing for is that CCI does not use plain multiple choice throughout. It mixes traditional items, where one of four options is correct, with what it calls innovative multiple response items, where two or three answers are correct out of four to six options and you must select all of them. Partial knowledge that gets you through a four-option question falls apart on a select-all-that-apply item, because there is no credit for getting most of it right. If every practice question you have ever done had exactly one right answer, you have not rehearsed the harder half of the format.
Neither passing score is a percentage, and CCI is unusually blunt about this: its own guidance states that CCI scaled scores do not equal the number or percentage of questions answered correctly, and that the number of correct answers needed to reach 650 was established through a standard-setting study by panels of subject matter experts. NHA set its 390 the same way. One figure from NHA's standard-setting study is worth knowing given how little pass rate data exists in this field: at the established cut score, the pass rate for the initial sample of 252 candidates was 73.8%. That is a snapshot from a standard-setting sample rather than a current national rate, so read it as calibration, not as your odds.
How to study for the one you picked
For the CRAT and the CCT, volume on strips, and one fixed reading sequence you never deviate from. Rate, regularity, P waves and whether each produces a QRS, PR interval, QRS width. Five checks in the same order every time, so that under a 55 second clock you are running a routine instead of hunting for a pattern you half remember. Test writers build hard items by giving you two rhythms that differ in exactly one measurement, sinus bradycardia against a junctional rhythm, Mobitz I against Mobitz II, and then seeing whether you checked it. A fixed sequence is what makes you check it.
For the CET, deliberately spend most of your time outside the strips, because that is where 76 of the 100 scored items live. Lead placement to the exact intercostal space, artifact identification and what fixes each type, skin preparation, calibration and standardization, HIPAA, infection control and scope of practice. The scope of practice items in particular are worth rehearsing, because the wrong answer usually feels kinder than the right one: telling a worried patient the tracing looks fine is interpretation, and it is outside your role no matter how confident you are.
That division of labor is not unique to cardiology. Across diagnostics the technologist acquires the study and a physician signs it out, which is why imaging departments run software that flags suspected findings and prioritizes the worklist for the radiologist rather than issuing a report on its own. Understanding where your responsibility stops is part of the job description, and all three of these exams test it directly.
Whichever credential you registered for, practice it in its own proportions. Is an EKG certification worth stacking with others?
For a monitor tech who intends to stay in cardiac care, yes, and the natural next step is nursing or a progressive care role rather than another technician credential. Techs who go that route commonly end up sitting the PCCN progressive care exam or, in an intensive care unit, the CCRN, both of which lean hard on the rhythm interpretation you already built. The CRAT is a genuinely useful foundation for both, which is not something you can say about most entry-level credentials.
For someone using an EKG credential as a first step into healthcare generally, the CET is the more sensible purchase, because it is the one training programs bundle and the one that signals entry-level competence across the full technician role rather than depth in one slice of it. Then, once you are employed and can qualify through the CCI work experience pathway, the CRAT costs $175 and no tuition.
