The short answer

There are two sterile processing technician certifications in the United States and they are not the same exam. The HSPA CRCST and the CBSPD CSPDT are both 150 questions, 125 scored plus 25 unscored pretest items, taken in three hours, but in 2025 the CRCST passed 65.6% of 15,369 candidates while the CSPDT passed 48.1% of 4,004. CBSPD publishes a passing score of 70 and HSPA publishes none at all, because HSPA sets its passing point with the Angoff procedure supplemented by the Beuk Relative-Absolute Compromise method. The difference that decides most people is eligibility: HSPA will let you sit the exam before you have any experience and submit your 400 hands-on hours within six months of passing, while every CBSPD route requires 12 months of work or a completed training course before the application deadline.

Dried bioburden has to be softened and lifted before any mechanical process can work on it. Both the ultrasonic cleaner and the washer disinfector assume gross soil is already gone, and neither reliably removes protein that has dried inside a box lock. Soaking in an enzymatic solution mixed at the dilution and temperature the manufacturer specifies is what makes the later stages effective. This is also the argument for point-of-use treatment: keeping instruments moist in the operating room prevents the problem instead of solving it, and a tray that arrives dry is something you report rather than absorb.

Heat sets protein the way it sets an egg white. Blood and tissue that meet hot water bond to the surface, become much harder to remove, and can shield microorganisms from the sterilant downstream. Manual cleaning therefore uses cool or lukewarm water at the temperature the detergent manufacturer states. The same reasoning is behind two other rules the exam tests: instruments must not be allowed to dry before decontamination, and saline is not an acceptable holding solution because it is corrosive to stainless steel.

The Bowie-Dick is an air removal test for dynamic air removal, or prevacuum, sterilizers. Residual air keeps steam from contacting every surface, so a load can run a complete cycle and still not be sterile. It is run in an otherwise empty chamber at the start of each day the sterilizer will be used, before any processed load. It does not test lethality, which is the biological indicator's job, and it does not apply to gravity displacement sterilizers, which remove air a different way. A failed Bowie-Dick takes the sterilizer out of service until the cause is found.

A wet pack is contaminated. Moisture makes a continuous path from the outside of the wrapper to the contents, so microorganisms travel in by capillary action, which is called strike-through. The tray is unwrapped, cleaned as required, repackaged and sterilized again. Replacing the wrapper or drying the pack does not fix it, because contamination is presumed to have already happened. Wet packs are also a signal rather than an isolated event, so the cause gets investigated: overloading, dense sets, poor load configuration, insufficient dry time or steam quality.

Preparation and packaging is a clean area, and an item that fails inspection there goes back rather than forward. Anything with visible soil, staining or residue is treated as not clean, and decontamination is the only correct destination. Wiping it in the clean area contaminates the assembly surface and hides a cleaning failure that should be investigated. Inspection at this point is a real control, not a formality: it is the last opportunity to catch a problem before an instrument is packaged, sterilized and taken to a sterile field.

An internal chemical indicator responds to conditions inside the package and shows that the sterilant reached that location. It does not prove sterility. Only a biological indicator challenges the process with live spores, and only that demonstrates lethality. The three-part system is what the exam tests: the external indicator separates processed from unprocessed packages, the internal indicator shows penetration to its own position, and the biological indicator verifies the kill. A failed internal indicator means the set is not used, whatever the tape on the outside looks like.

Standards such as ANSI/AAMI ST79 and the AORN guidelines describe how to build and run a program, and facility policy translates them into local practice, but none of them can override a validated instruction for a particular device. This principle runs through the whole content outline, in chemical handling, equipment operation and sterilization parameters, and it is the answer to a surprising number of scenario questions that look like they are about something else. CRCST or CSPDT: which sterile processing certification should you sit?

Two organizations certify sterile processing technicians in the United States and employers largely treat them as interchangeable, which is why almost nobody compares them before applying. They are worth comparing, because the two exams differ in ways that change your odds and your timeline. HSPA, the Healthcare Sterile Processing Association, was called IAHCSMM until 2021 and has been certifying technicians since 1971. It awards the CRCST, the Certified Registered Central Service Technician, and as of December 31, 2025 there were 54,525 current CRCSTs against a cumulative 153,765 exams administered since the first sitting in 1975. CBSPD, the Certification Board for Sterile Processing and Distribution, awards the CSPDT, the Certified Sterile Processing and Distribution Technician, and had 18,353 current holders. So HSPA is roughly three times the size, and in 2025 it tested 15,369 candidates against CBSPD's 4,004. Both credentials are NCCA accredited, and the CRCST also carries ANAB accreditation. Both exams are 150 multiple-choice questions with 125 scored and 25 unscored pretest items, both allow three hours, and both are offered only in English. From there they diverge. HSPA tests year-round through Prometric and will let you sit the exam before you have any experience at all. CBSPD tests in four one-week windows a year, takes a paper application by mail, and requires you to already have 12 months of work or a completed training course before the deadline. If you are trying to get into the field rather than certify a job you already hold, that single difference usually settles it.

What is the passing score for the CRCST exam?

HSPA does not publish one. Its certification handbook states that the passing points for the CRCST, CIS, CER and CHL exams were established using a criterion-referenced technique, specifically the Angoff procedure supplemented by the Beuk Relative-Absolute Compromise method, in which content experts estimate the probability that a minimally competent candidate answers each question correctly, and psychometricians turn those estimates into a recommended cut score that the Certification Council approves. What HSPA does tell you is the mechanics: one point for every correct answer, zero for an incorrect one, no penalty for guessing, and your pass or fail decision made on the number of the 125 scored questions you get right. The 25 pretest questions are indistinguishable from the scored ones and do not count, which is the practical reason to answer every item. CBSPD is different and states its standard openly: a passing scaled score of 70 on the CSPDT. That number is where the "CRCST is 70%" claim came from, and it has been copied onto CRCST prep sites for years. Treat any specific CRCST percentage you see online as unsourced. A criterion-referenced cut score is also not a fixed percentage by design, because it is tied to the difficulty of the particular form you sat rather than to a round number chosen in advance. Study to cover the content, not to clear a threshold nobody has published.

Is the CRCST exam hard? The published pass rates say it is getting harder

HSPA publishes pass and fail counts for every certification, every year, on its certification statistics page. Almost no prep site uses them. They show something a prospective candidate should know: the CRCST pass rate has fallen in each of the last three years, from 74.9% in 2022 to 71.9% in 2023, 66.9% in 2024 and 65.6% in 2025. The peak was 77.4% in 2019. Over the same period the number of people sitting the exam nearly doubled, from 8,289 in 2019 to 15,369 in 2025. In 2025 alone, 5,286 candidates failed the CRCST, which is more people than sat the CSPDT in total that year. The likeliest reading is straightforward and does not require a conspiracy about the exam getting harder. Sterile processing has been hiring hard, a lot of those new candidates are testing early in their careers, and the provisional route explicitly invites people to sit the exam before they have their 400 hours. A larger, greener candidate pool produces a lower pass rate without the questions changing at all. The CBSPD numbers point the same way from a different starting point. The CSPDT passed 63% in 2020, then 56%, 48%, 50%, 50.4% and 48.1% in 2025. Both credentials peaked around 2019 to 2020 and have settled lower. What this means for you is simple: roughly one candidate in three fails the CRCST and roughly one in two fails the CSPDT, so the exam is not a formality you clear by showing up with a year in the department.

How many questions are on the CRCST exam, and how are they weighted?

One hundred and fifty multiple-choice questions in three hours, of which 125 are scored and 25 are unscored pretest items seeded through the exam to be trialled for future forms. The test is computer-based at a Prometric center, there is no written version, questions can be marked and returned to, and a 15-minute tutorial runs before the exam with the clock starting afterwards. The part that should shape your study plan is the weighting, published in the CRCST Exam Content Outline revised in November 2023 off the back of that year's job task analysis. Three of the seven domains carry 21% each: cleaning, decontamination and disinfection; preparation and packaging; and the sterilization process. Together that is 63% of the exam, or roughly 79 of the 125 scored questions. Departmental considerations takes 15%. The remaining three domains, sterile storage and transport and inventory management at 9%, professional development and human relations at 8%, and patient care equipment and distribution at 5%, add up to 22% between them, or about 28 questions. Most CRCST study plans work chapter by chapter through a manual and give every chapter about the same attention. That is a bad trade. Patient care equipment is a real part of the job and a small part of the exam, and an hour spent there is worth about a third of an hour spent on the sterilization process.

Can you take the CRCST exam without experience? The provisional route explained

Yes, and this is the most useful thing HSPA offers someone trying to break into the field. Full CRCST certification requires 400 hours of hands-on experience in a sterile processing department within the previous five years, documented on the application by your supervisor. The provisional route inverts the order: you sit and pass the exam first, then have six months to complete and submit those 400 hours. If the hours do not arrive inside the window, the provisional certification is forfeited and you have to retake the exam, though a one-time two-month extension is available if you are currently working or volunteering in a department and ask before your provisional status expires. The 400 hours are not a lump you can fill however you like. HSPA breaks them into decontamination 120 hours, preparation and packaging 120 hours, sterilization and disinfection 120 hours, storage and distribution 24 hours, and quality assurance processes 16 hours. Hours can be paid or volunteer, and you do not need to be currently employed to test. They must be signed by someone in a position above yours who has physically watched you do the work: a lead technician, coordinator, supervisor, manager, chief, director, administrator or hospital-based educator. Technicians at your own level and private instructors cannot sign, and you may never document your own hours. HSPA verifies a sample of applications, and if yours is selected your supervisor has to be reachable, so keep their contact details current or the application is rejected. CBSPD offers no equivalent. Every one of its four eligibility routes wants something completed before the application deadline: 12 months of full-time SPD employment, or six months each of allied health work and SPD work, or a sterile processing technician training course passed at 70 or higher, or 12 months in healthcare product sales or service related to the profession.

How much is the CRCST exam, and what does it cost to hold the credential?

The CRCST application fee is $140 and it covers processing plus one exam attempt at a Prometric center. A retake is another $140, with no discount for having sat it before. Cancellations and refunds are only possible within 30 days of payment and come back minus a $25 non-refundable application fee. The CSPDT costs $135, with convenience fees possible on top. So the entry price is within five dollars either way and is not the thing to decide on. The ongoing cost is where they part company, and it runs in the opposite direction to what most people assume. HSPA renews annually: 12 continuing education credits every year and a $50 renewal fee, which covers all your HSPA certifications however many you hold, or $60 if you add membership. Over five years that is $250 in fees and 60 CE credits, on five separate deadlines. CBSPD certifies for five years at a time and asks for 100 or 150 points across the whole cycle depending on the credential, or a retest, on one deadline. The two organizations count continuing education differently and the units are not interchangeable, so the honest comparison is about cadence rather than volume: HSPA asks for a little every year and charges every year, CBSPD asks for a lot once. One more cost is worth naming because HSPA names it itself. Its site carries an explicit warning that courses offered by third-party providers and academic institutions are not required prerequisites for certification, that nobody needs to enroll in one to sit the exam, and that candidates should be vigilant against organizations charging high prices while claiming otherwise. You can apply directly, study on your own, and pay $140.

How to study for the CRCST exam when three domains carry 63% of it

Start by mapping your effort to the published weights rather than to the chapters of whatever manual you bought. If you have twenty hours to spend, about twelve and a half of them belong to decontamination, preparation and packaging, and sterilization, three hours to departmental considerations, and the remaining four and a half split across storage, patient care equipment and professional development. That allocation alone puts you ahead of most candidates. Second, learn the exam's underlying logic instead of memorizing lists. A large share of CRCST scenario questions resolve to the same answer: follow the manufacturer instructions for use. The IFU outranks the facility policy, and it outranks your own habit, because it is the only document validated for that specific device. When a question describes a conflict between what the department always does and what the IFU says, the IFU wins. Third, practice under conditions that resemble the exam. Three hours for 150 questions is 72 seconds each, which is comfortable, so the failure mode is not time pressure but second-guessing. Get used to marking an item for review and moving on rather than sitting on it. Fourth, treat a question you got right by eliminating two options and guessing between the last pair as a question you got wrong, and come back to it. A rephrased version of that item will not go your way. And finally, use the reference set HSPA actually built the exam from: the SP Technical Manual 9th edition from 2023, ANSI/AAMI ST79 from 2017, and the AORN Guidelines for Perioperative Practice from 2023. If a study aid predates those, it predates the current exam.

Cleaning and decontamination: 21% of the exam and the source of the trick questions

This domain covers the dirtiest end of the workflow and it is where the exam most often tests sequence rather than fact. The content outline runs through area-specific safety, OSHA and bloodborne pathogens, microbiology including cross contamination and biofilm and Creutzfeldt-Jakob disease, personal protective equipment and how it is donned, doffed and changed, safe transport of soiled items including inspecting for and reporting inadequate point-of-use cleaning, cleaning tool selection, equipment operation for washer disinfectors and ultrasonics and cart washers and leak testers, and chemical identification and preparation following the IFU. A few principles answer a disproportionate number of items. Cleaning always precedes disinfection and sterilization, and no process downstream can compensate for soil left upstream, because organic material physically shields microorganisms from the sterilant. Protein coagulates in heat, which is why manual cleaning uses cool or lukewarm water at the detergent manufacturer's stated temperature and why instruments must not be allowed to dry before decontamination. An ultrasonic cleaner works by cavitation, so instruments go in opened or disassembled and the tank is degassed after filling, because dissolved air stops the bubbles forming properly. Point-of-use treatment in the operating room is part of your process even though you are not the one doing it, and a tray that arrives with dried soil is something you report, not something you quietly deal with. Airflow and pressure relationships matter here too: decontamination is a negative pressure area so that its air does not travel to the clean side, and traffic runs one way from dirty to clean.

Preparation and packaging: inspection, indicators and the assembly sequence

Another 21%, and this is the clean side of the barrier. The recurring theme is that preparation and packaging is the last chance to catch a cleaning failure, so an instrument that fails inspection there goes back to decontamination rather than forward. Wiping a stain in the clean area is wrong twice over: it contaminates the assembly surface and it hides a process failure that should be investigated. Instrument inspection covers function as well as cleanliness, so scissors that will not cut test material, needle holders with worn jaws, hinges that bind and insulation defects on electrosurgical instruments all come out of the set. The indicator questions in this domain are worth learning as a three-part system, because the exam tests the distinction rather than the definitions. An external chemical indicator, usually the tape or the printed pattern on a pouch, tells you a package has been through a sterilizer and nothing more, so it separates processed from unprocessed items. An internal chemical indicator placed inside the package responds to the conditions at that location and tells you the sterilant penetrated to that point. A biological indicator challenges the process with live spores and is the only one of the three that demonstrates lethality. A pass on the tape means nothing about what happened inside the pack, and a failed internal indicator means the set is not used no matter how good the tape looks. Packaging itself is governed by the sterilization method you intend to use, the IFU for the device and the IFU for the packaging material, which have to agree with each other.

The sterilization process: parameters, monitoring and what a failed indicator means

The third 21% domain. Steam is the default because it is fast, reliable, non-toxic and cheap, and the exam expects you to know why a device would be sent to a low-temperature method instead, which is heat or moisture sensitivity stated in the IFU rather than a judgment call you make. Monitoring is the heart of this domain and the questions divide neatly into three kinds. Physical monitoring is the printout and the gauges: time, temperature and pressure, checked and initialed for every load. Chemical monitoring is the indicator system described above. Biological monitoring is the spore test, and the organism pairing is a standard exam item: Geobacillus stearothermophilus for steam and hydrogen peroxide, Bacillus atrophaeus for dry heat and ethylene oxide. Learn them as a pair rather than separately. The Bowie-Dick test is its own question type and is frequently confused with a biological indicator. It tests air removal and steam penetration in a dynamic air removal, or prevacuum, sterilizer, it is run in an otherwise empty chamber at the start of each day the sterilizer is used, and it does not test lethality. Residual air is the point: a load can complete a full cycle and still not be sterile because steam never contacted every surface. Wet packs are the other high-frequency item. A pack that comes out visibly wet is treated as contaminated, because moisture creates a path from the outside of the wrapper to the contents, which is strike-through. It is fully reprocessed, not dried on a cart, and the cause is investigated, because overloading, dense sets, poor load configuration, short dry time and steam quality all produce wet packs and all are fixable. Immediate use steam sterilization, IUSS, appears in the content outline under quality assurance benchmarks for a reason: it is monitored precisely because it is the exception rather than a routine workflow.

Departmental considerations: the 15% most candidates underprepare

This is the largest single domain after the three processing domains and it is the one people skip, because it feels like administration rather than technique. It is worth about 19 of the 125 scored questions, which is more than storage and patient care equipment combined. It covers five areas. Environmental considerations: work-area-specific temperature, humidity and airflow requirements per ASHRAE guidelines, what corrective action to take when those parameters drift out of range, area-specific cleaning protocols, and chemical safety including how to read an IFU and a safety data sheet, spill protocol, disposal and eyewash stations. Personnel considerations: dress code, hand hygiene, surgical attire, artificial nails and jewelry, traffic control and one-way workflow, work area classifications and ergonomics. Regulations and standards: the difference between a regulation and a standard, which is the distinction the exam actually cares about. FDA and CMS write regulations that carry legal force. ANSI/AAMI and AORN publish standards and guidelines that become enforceable when a facility adopts them or an accrediting body expects them. Documentation and record keeping: what records exist, how long they are retained, where they live, and why, which is partly standards compliance and partly that these are legal documents. And quality assurance: benchmarks like tray audits, IUSS rates, instrument set turnaround times, case cart accuracy and inventory fill rates, plus the improvement frameworks by name, total and continuous quality improvement, Six Sigma and LEAN. Reading that list once is worth more than a third pass through decontamination.

The CBSPD CSPDT exam, and why its four windows a year change your plan

If you are sitting the CSPDT rather than the CRCST, the exam itself will feel familiar: 150 questions, 125 scored and 25 unscored pretest items, three hours, a passing scaled score of 70, and certification for five years. The logistics are the part to plan around. CBSPD runs four testing windows a year, each one week long. In 2026 they were February 2 to 7, May 4 to 9, August 3 to 8, and November 2 to 7, with application deadlines the Tuesday before each window: January 27, April 28, July 28 and October 27. Applications go up on the CBSPD site roughly 30 to 40 days before the window opens, and they cannot be completed online. You print the application, complete it, and mail it with payment to the CBSPD office in Lebanon, New Jersey. Once accepted you get an email or postcard telling you how to schedule your date and site inside the window, and CBSPD asks you to contact the office if that has not arrived seven days before the window. Results are mailed about three weeks after the window closes. You may only take the same exam once in a window, so a failure means waiting for the next one, which is roughly three months. Compare that to HSPA: apply online, allow three to four weeks for processing, then schedule any date at any Prometric center inside a 90-day eligibility window, with pass or fail on screen when you finish and an email confirming it within 48 hours. If your employer wants you certified by a date, the CBSPD calendar is the constraint you have to work backwards from.

How to pass the CRCST exam: results, the score report and the six-week retake

You find out at the test center. HSPA gives an on-screen pass or fail notification once you finish the exam and the survey, emails the same result to the address you gave Prometric within 48 hours, and mails a certification package with your certificate inside two weeks if you passed. If you did not, the score report breaks your performance down by knowledge domain so you can see where to work. Read that report carefully but do not over-read it. HSPA states plainly that the domain scores are not used to determine pass or fail, that they are a general indication only, and that they should be interpreted with caution, because the exam is designed to measure overall competence precisely rather than to measure each domain precisely. A weak-looking domain with eight questions in it is a small sample. Use the report to choose between two or three plausible study priorities, not to rebuild your whole plan around the lowest bar. The retake rule is a mandatory six-week wait. You can submit a retake application as soon as you like, but the new eligibility period does not open until six weeks after your last exam date, and the fee is the full $140 again. Two operational traps are worth knowing because they end your eligibility and forfeit the fee without you ever sitting the exam. The first is the 90-day window: you must schedule and take the exam inside it, and missing it means reapplying and repaying. The second is identification. Prometric requires one current, non-expired photo ID bearing your signature and matching the name on your application exactly. A name change from marriage or divorce, or a typo, has to be fixed with HSPA before you schedule, not on the day.

How to renew CRCST certification, and what an expired card costs you

The CRCST renews every year, which surprises people coming from credentials that run on three-year or five-year cycles. You need 12 continuing education credits a year, focused on the sterile processing field and technical in nature, plus a $50 renewal fee. That fee covers all your HSPA certifications no matter how many you hold, so a technician who also holds the CIS or the CER pays the same $50, and adding HSPA membership makes it $60. Credits accrue in hour increments, and they can be quarter or half credits, so small activities count. The four categories HSPA recognizes are lesson plans, webinars and podcasts, which its own materials supply at 2 CEs for a lesson plan with a passed quiz, 1 CE for a webinar with a completed survey and half a CE for a podcast episode with a quiz; in-services and departmental meetings relevant to sterile processing, submitted on the HSPA form or on facility letterhead with the date and topic; and the other two categories described on the CE submission form. Vendor and association certificates count as long as the printout shows your name, the completion date, the topic and the CE value, which is worth checking at the time rather than a year later. The practical advice is to submit as you go. Twelve credits is a modest requirement spread over twelve months and a painful one to assemble in December from certificates you cannot find. CBSPD works the opposite way: 100 or 150 points across a five-year cycle depending on the credential, or a retest, with recertification packets downloadable from its site by the year in which you are due.

HSPA does not publish one. Its certification handbook states that the passing point is criterion-referenced, set with the Angoff procedure supplemented by the Beuk Relative-Absolute Compromise method, and it publishes no percentage. The 70% you see quoted on prep sites is CBSPD's published passing scaled score for its CSPDT exam, which is a different credential from a different organization.

One hundred and fifty multiple-choice questions, of which 125 are scored and 25 are unscored pretest items seeded through the exam. You get three hours. The pretest items cannot be told apart from the scored ones, so answer everything, and there is no penalty for a wrong answer. The CBSPD CSPDT uses exactly the same structure: 150 questions, 125 scored, 25 pretest, three hours.

Harder than its reputation, and getting harder in the numbers. HSPA's own statistics show the CRCST pass rate falling from 77.4% in 2019 to 65.6% in 2025, so about one candidate in three now fails. In 2025, 5,286 people failed the CRCST. The CBSPD CSPDT is harder still on the published figures, passing 48.1% in 2025.

The HSPA application fee is $140, which covers processing and one attempt at a Prometric center, and a retake is another $140. Refunds are only available within 30 days of payment and come back minus a $25 non-refundable application fee. The CBSPD CSPDT is $135 plus possible convenience fees.

Yes, through HSPA's provisional route. You sit and pass the exam first, then have six months to complete and submit 400 hours of hands-on experience in a sterile processing department. Miss that window and the provisional certification is forfeited and you retake the exam, though a one-time two-month extension exists if you are currently working in a department and ask before it expires. CBSPD has no equivalent.

Allow three to four weeks for HSPA to process your application, then you have a 90-day window to schedule and sit the exam at a Prometric center. You get pass or fail on screen at the end and an email within 48 hours, and a certification package arrives within two weeks of passing. If you are on the provisional route, full certification follows once your 400 hours are submitted and approved.

One year. The CRCST renews annually with 12 continuing education credits and a $50 fee, which covers all your HSPA certifications however many you hold, or $60 with membership added. This catches people out, because the CBSPD CSPDT runs on a five-year cycle instead, asking for 100 or 150 points across the whole period or a retest.

There is no lifetime cap, but there is a mandatory six-week wait between attempts. You can submit a retake application whenever you like, and your new eligibility period opens six weeks after your last exam date. Each attempt costs the full $140. CBSPD works differently: you may only sit the same exam once per one-week window, so a failure means waiting about three months for the next one.

Weight your study to the published content outline rather than working through a manual chapter by chapter. Cleaning and decontamination, preparation and packaging, and the sterilization process are 21% each, so three domains carry 63% of the exam, about 79 of the 125 scored questions. HSPA built the current exam from the SP Technical Manual 9th edition (2023), ANSI/AAMI ST79 (2017) and the AORN Guidelines for Perioperative Practice (2023).

No, and HSPA says so on its own site. It warns explicitly that third-party and academic courses are not prerequisites for certification, that enrollment is not mandatory, and that candidates should be cautious about organizations charging high prices while claiming a course is required. You can apply directly, prepare on your own, and pay the $140 exam fee.

Employers generally accept either, and both are NCCA accredited, so the choice is practical rather than about prestige. HSPA is about three times larger, tests year-round, takes an online application, and will let you sit the exam before you have any experience. CBSPD tests in four one-week windows a year, needs a paper application by mail, requires completed work or a course before the deadline, and certifies for five years instead of one.
