Three different organizations certify EKG technicians in the United States, and they do not test the same thing. The NHA Certified EKG Technician exam is 100 scored items in 2 hours, reported on a 200 to 500 scale with a passing scaled score of 390, and EKG analysis and interpretation accounts for only 24 of those 100 items. The CCI Certified Cardiographic Technician and Certified Rhythm Analysis Technician exams are each 130 questions, 110 of them scored, also in 2 hours, reported on a 0 to 900 scale with a passing scaled score of 650. On the CCT, rhythm analysis and 12-lead analysis together are 62% of the score. On the CRAT, analyzing normal and abnormal rhythms is 76%. Question 1 · Rhythm Analysis
A rhythm strip shows a regular rhythm at 46 beats per minute. Every P wave is followed by a QRS, the PR interval measures 0.18 seconds and does not change, and the QRS is 0.08 seconds. The correct interpretation is:
- A. First degree AV block
- B. Sinus bradycardia
- C. Junctional rhythm
- D. Second degree AV block, Mobitz I

Run the five checks and the answer falls out. Every P conducts, the PR interval is inside the normal 0.12 to 0.20 second range and stays constant, and the QRS is narrow, so conduction is normal throughout. The only abnormal finding is a rate under 60. A first degree block would need a PR interval longer than 0.20 seconds. Mobitz I requires progressive PR lengthening and a dropped beat. A junctional rhythm would not show an upright P wave before each QRS in lead II.
Question 2 · Rhythm Analysis
A monitored patient suddenly shows a chaotic baseline with no identifiable P waves, QRS complexes or T waves, and varying amplitude. The patient is unresponsive with no pulse. This is:

The waveform alone does not settle this, which is the whole point of the item. Loose lead artifact can look identical on the screen. What decides it is that the patient is unresponsive and pulseless, and ventricular fibrillation produces no cardiac output. This is why every protocol says look at the patient before you troubleshoot the monitor. Atrial fibrillation still shows discrete QRS complexes, and asystole is a flat line rather than a chaotic one.
Question 3 · Rhythm Analysis
Two strips both show a slow rate with a narrow QRS and dropped beats. On the first, the PR interval lengthens progressively before a QRS is dropped. On the second, the PR interval is constant and a QRS drops without warning. The first and second strips show, respectively:

A constant PR interval with an abruptly dropped beat is Mobitz II, which reflects disease below the node and is the more dangerous of the two because it progresses to complete block. In third degree block the P waves and QRS complexes are entirely dissociated and march at independent rates, with no relationship between them to measure.

A tracing shows a fine, irregular, fuzzy oscillation of the baseline in all leads while the patient shivers in a cold room. The cause and the correct first action are:

Muscle activity produces somatic tremor, an irregular fuzzy artifact whose severity tracks how tense the patient is, and shivering is the textbook trigger. The fix addresses the patient, not the machine. Sixty cycle interference is a perfectly regular thick band from an electrical source. A wandering baseline drifts slowly and points to loose or dried electrodes or to respiration. Lead reversal changes the shape and polarity of complexes rather than adding noise.

A tracing shows the standardization mark at 5 mm instead of 10 mm. This means:

Standard calibration is 10 mm of deflection per 1 millivolt, so a 5 mm mark means the gain was halved, which is sometimes done deliberately when complexes are so tall they overlap each other. The practical consequence is that you cannot judge voltage on any tracing without reading the standardization mark first: a halved gain mimics low voltage and a doubled gain mimics hypertrophy. Question 7 · Safety and Scope of Practice
A patient asks the EKG technician what the tracing shows and whether he has had a heart attack. The appropriate response is to:

Interpreting a tracing for a patient falls outside the EKG technician scope of practice, and scope questions appear on all three of these exams. The correct handling is neither to interpret nor to stonewall. Reassuring a patient that a tracing looks normal is interpretation, and it is the most commonly chosen wrong answer because it feels kind. Walking out without acknowledging the question fails the patient care half of the item.

Under HIPAA, an EKG technician may discuss a specific patient tracing with:

Being present in the room does not authorize disclosure to a family member, because the patient controls that permission. A second opinion from a colleague who is part of the care team is fine, and the same conversation with a colleague who has no role in the case is not.

Hair holds the gel off the skin, which produces a wandering baseline and poor signal that no amount of pressure corrects. Standard skin preparation is to clip rather than shave, after telling the patient what you are doing, then clean and lightly abrade the site. Moving a chest lead off its anatomical landmark is never acceptable, because the landmark is what gives that lead its meaning.

It depends which exam, and neither passing score is a percentage. The NHA Certified EKG Technician exam is reported on a scaled score range of 200 to 500 with a passing score of 390. The CCI exams, the Certified Cardiographic Technician and the Certified Rhythm Analysis Technician, are reported on a scale of 0 to 900 with a passing score of 650. CCI is unusually direct about what that means. Its own guidance states that CCI total scaled scores do not equal the number or percentage of questions answered correctly, and that the number of correct answers needed to reach 650 was set through a standard-setting study by panels of subject matter experts. So there is no fixed number of questions you have to get right. It moves with the difficulty of the form you draw. NHA set its 390 the same way, through a modified Angoff procedure in which a panel judged each item against a minimally qualified candidate. One number does survive from that study and it is worth knowing, because published pass rates barely exist in this field: NHA reported that at the established cut score, the pass rate for the initial sample of 252 candidates was 73.8%. That is a standard-setting sample from one point in time, not a current national pass rate, and it should be read as a rough calibration rather than your odds.
How many questions are on the EKG certification exam and how long is it?
Both CCI exams run 130 questions in two hours, of which 110 are scored and 20 are unscored pretest items seeded to gather statistics for future forms. You cannot tell which are which, so the only workable strategy is to answer everything as though it counts. Two hours across 130 items is about 55 seconds per question. The NHA CET test plan specifies 100 scored items with a 2 hour exam time. NHA embeds unscored pretest items as well, and its own practice analysis report for the CET describes each test as comprising 100 scored items plus 10 unscored pretest items. Many prep sites report 20 pretest items instead. The scored count and the clock are the parts that affect you, and both of those come straight from NHA test documentation. One format difference genuinely changes how you should practice. CCI does not use plain multiple choice throughout. It mixes traditional items, where one of four options is correct, with what it calls innovative multiple response items, where two or three answers are correct out of four to six options and you have to select all of them. Partial knowledge that would carry you through a four-option question does not carry you through that. If your practice material is entirely single-answer multiple choice, you have never rehearsed the harder half of the CCI format.
Which EKG certification should you take: CET, CCT or CRAT?
Start from the job rather than the acronym, because these three credentials describe genuinely different work and the exams reflect that. The NHA Certified EKG Technician is the entry-level credential most commonly attached to a 4 to 12 week EKG technician program, and its exam looks like an entry-level allied health exam. EKG acquisition is the largest domain at 44 of the 100 scored items: hooking the patient up correctly, getting a clean tracing, troubleshooting artifact. Safety, compliance and coordinated patient care is 32 items. Analysis and interpretation is 24. It is a test about doing the procedure properly. The CCI Certified Cardiographic Technician is aimed at professionals already working in ECG, Holter monitoring and stress testing, and it is a much heavier interpretation exam. Rhythm analysis alone is 40% of the score and analyzing the 12-lead ECG is another 22%, so 62% of the exam is reading tracings. Performing stress tests is 10% and ambulatory monitoring is only 4%, which surprises people who assume a Holter-heavy job produces a Holter-heavy exam. The CCI Certified Rhythm Analysis Technician is the monitor tech and telemetry credential, and it is the most concentrated of the three. Analyzing abnormal rhythms is 60% of the score on its own. Add analyzing normal rhythms at 16% and three quarters of the exam is rhythm interpretation. If you sit in front of a bank of telemetry screens, this is your exam, and a general EKG study guide will underserve it badly. A practical note on sequencing: many people take the NHA CET first because a training program includes it, then add a CCI credential once they are employed and can qualify through the work experience pathway. That order works, but do not assume the CET prepared you for the CCT. It prepared you for 24% of it.
What do you actually have to know to pass the rhythm portion?
The same core list, on all three exams, and it is shorter than the internet suggests. Sinus rhythm, sinus bradycardia, sinus tachycardia and sinus arrhythmia. Atrial fibrillation, atrial flutter, supraventricular tachycardia, premature atrial contractions and wandering atrial pacemaker. Junctional rhythms, including accelerated junctional and junctional tachycardia. Premature ventricular contractions and their patterns, bigeminy, trigeminy, couplets, ventricular tachycardia, ventricular fibrillation, idioventricular and accelerated idioventricular rhythm, and asystole. First degree AV block, both types of second degree block, and third degree block. Paced rhythms, atrial, ventricular and dual chamber, plus failure to capture and failure to sense. Bundle branch blocks. What separates people who pass from people who nearly pass is not the length of that list. It is having one fixed sequence you run on every strip, so that under time pressure you are executing a routine rather than pattern matching from memory. Rate, then regularity, then P waves and whether each one is followed by a QRS, then the PR interval, then the QRS width. Five checks, always in the same order, and the answer falls out of them. That sequence is also what defends you against the distractors these exams are built from. Sinus bradycardia and a junctional rhythm both run slow with a narrow QRS, and the only thing that separates them is the P wave. Second degree Mobitz I and Mobitz II both drop beats, and the only thing that separates them is whether the PR interval was lengthening beforehand. A test writer who wants to make an item hard does not invent an exotic rhythm. They give you two rhythms that differ in exactly one measurement and see whether you checked it.
How do you read an EKG strip step by step?
Begin with the paper, because every interval measurement depends on it. At the standard speed of 25 mm per second, one small box is 0.04 seconds and one large box, five small boxes, is 0.20 seconds. Five large boxes make one second, and 15 large boxes make the 3 second interval marked on most strips. Check the standardization mark too: it should be 10 mm tall for a 1 millivolt signal, and a half-height mark means the gain was reduced, which will make normal voltage look low. For rate on a regular rhythm, divide 300 by the number of large boxes between two R waves, which is why the sequence 300, 150, 100, 75, 60, 50 is worth memorizing outright. For an irregular rhythm that method fails, so count the QRS complexes in a 6 second strip and multiply by 10. Then run the five checks. Regularity: march the R to R intervals and see whether they stay constant. P waves: is there one before every QRS, is it upright in lead II, and does every P wave produce a QRS? PR interval: normal is 0.12 to 0.20 seconds, and you are watching both its length and whether it stays the same beat to beat. QRS width: normal is under 0.12 seconds, and wide means the impulse either started in the ventricles or took a detour through a blocked bundle. The habit that matters most on exam day costs two seconds and prevents the most common wrong answer: before you interpret anything, ask whether what you are looking at could be artifact. Somatic tremor from a shivering or tense patient produces an irregular fuzzy baseline. Sixty cycle interference produces a perfectly regular thick line from an electrical source. A wandering baseline drifts slowly and usually means a dried or loose electrode. Each of these has been mistaken for a lethal rhythm by someone who skipped the question.
How much does EKG certification cost?
The CCI exams are the clearer number because CCI publishes it. Both the CCT and the CRAT cost $175, and CCI states that every exam fee includes a $100 non-refundable filing fee covering application review. That structure matters more than the headline price: if you cancel inside your first 90-day window you can request a refund, but you get back $75, not $175. Score verification, if you want your result rechecked, is another $50. NHA does not publish a CET fee on a page that automated checks can reach, and its site blocks them. Training programs that resell the voucher list it in the region of $129. Treat that as a secondary figure and confirm the current price with NHA or your program before you budget against it. Then there is the timing cost, which people underestimate. CCI issues an Authorization to Test with a 90 day window. You may request one extension, for another 90 days. Miss both and the fee is forfeited and you reapply from the start. Once you hold a CCI credential, it renews on a three year cycle with continuing education and a renewal fee listed at $165 in CCI documentation, and since the December 2016 renewal cycle CCI has moved to an attestation model rather than requiring you to submit CE documentation with every renewal.
Can you get EKG certified without a training program?
For the CCI credentials, yes, and this is the pathway most people miss. Both the CCT and the CRAT require a high school diploma or general education diploma, and then one of three qualification pathways. The first pathway is being currently enrolled in, or a graduate of, a cardiovascular or allied health training program, supported by a completion certificate, a student verification letter or transcripts. The second pathway is being currently or previously employed in cardiovascular technology or in an allied health field recognized by the American Medical Association, supported by an employment verification letter that states the period of employment, your primary duties and your clinical competence. The third is having completed a graduate or undergraduate degree in a science or physical health related field. The second pathway is the one that opens this to working monitor techs, nursing assistants and other allied health staff who never attended an EKG program. If your employer will write the letter, you can sit the exam. The NHA CET is structured differently, sitting inside NHAโs broader eligibility framework of a high school diploma or equivalent plus either a training program completed recently or documented supervised work experience. Confirm the current requirement with NHA directly, since NHA revises its handbook and does not expose it to automated checks.
How long does it take to get EKG certified?
The exam itself is two hours. Everything around it is what determines your timeline, and it splits cleanly by pathway. If you are going through a training program, the program is the schedule. EKG technician certificate programs at community colleges and vocational schools commonly run 4 to 12 weeks including a clinical component, and the certification exam is usually scheduled at or near the end. If you are qualifying through work experience, the clock is much shorter and it is administrative rather than educational. You gather your supporting documentation, apply through your CCI online account, pay, wait for CCI to review the application and issue your Authorization to Test, then schedule at a Pearson Professional Center subject to seat availability. Realistically, several weeks from deciding to sitting, most of it spent waiting on a verification letter from someone in your HR department. The study time is the variable you control. A working telemetry tech who reads strips every shift may need two or three weeks of targeted practice on the domains outside rhythm analysis. Someone coming from a classroom with no floor time usually needs longer, and needs it in the opposite place: the strips, at volume, until interpretation stops being deliberate and becomes automatic.

EKG acquisition is 44 and safety, compliance and coordinated patient care is 32. A candidate who studies only strips has prepared for under a quarter of that exam. On the CCI CRAT the same strategy would cover roughly three quarters of the exam, which is why the credential you registered for determines what you should study.
